CHPW advances integrated care by coordinating support at some of the most critical transition points in a member’s health journey, from reentry after incarceration to discharge from inpatient settings. By bridging gaps between systems, settings, and community-based services, CHPW helps members stay connected to care, stabilize their health, and move forward safely in their communities.
Supporting Healthier Reentry
In July 2025, CHPW launched Community Health Partnership Services (CHPS) after being awarded the Washington Health Care Authority’s contract to serve as the third-party administrator for state reentry services. Through Washington’s 1115 Medicaid Transformation Waiver, CHPS leverages our expertise in Medicaid managed care to help carceral facilities and provider partners, many of whom are new to Medicaid, navigate a set of reentry services including claims and billing. Importantly, CHPS also provides case management services, if needed, that are focused on stabilizing an individual’s health needs as they prepare to reenter their communities following incarceration.
By supporting facilities as they navigate Medicaid and managed care in nontraditional settings, CHPS enables earlier engagement, improved continuity of services, and stronger connections to community-based care.
To further strengthen this work and improve access to tools and information, CHPW launched the Community Health Partnership Services website at chpswa.org. The site provides centralized resources that support facilities in managing continuity of care, reducing preventable health risks, and supporting safer, more stable transitions back into the community.
