CHPW Community Health Workers Assist Members with Seattle Housing Authority Applications

CHPW Community Health Workers can help connect members to local resources, programs, and services for a variety of needs, including healthy foods, reliable transportation, community services, access to education, employment opportunities, access to childcare, and peer support.

Recently, our team spent time at Shoreline City Hall, where they assisted members with completing Seattle Housing Authority waitlist applications and connected members to additional food resources, clothing, and gas vouchers.

We never take it for granted that relationships are built by making connections. Connections with members, health navigators, neighbors, and community partners.

Sometimes, we can express that at clinic hours at CHCs, with provider partners, or at community events like this one.
