The 17th Annual Community Partnership for Transition Solutions (CPTS) Summer Institute in Wenatchee, Washington, brought together health care professionals with the shared goal of supporting successful transitions for people returning to our communities after incarceration.
On July 30 and 31, our local Community Health Plan of Washington team was in attendance to listen to engaging speakers like Tyron Nixon, the Medicaid Reentry Transformation Implementation Manager with the Washington State Health Care Authority, and participate in productive breakout sessions including the “Washington’s Managed Care Organizations Carceral Transitions Program” session.
CHPW was proud to be a Thriving Sponsor for the event in addition to providing scholarships to cover the event cost for 10 attendees. A special thank you to our Community Health Network of Washington Board Member and CEO of Family Health Centers, Jesus Hernandez, who led the event planning efforts this year, along with the conference host Advance Northwest.
CHPW Regional Manager Gerardo Perez-Guerrero was in attendance and shared, “Learning more about recovery coaches and the invaluable connection they offer to service providers was inspiring. Their willingness to share their lived experiences is essential to building trust with community members. I look forward to incorporating what I learned at the conference in my work at CHPW.”
CHPW recognizes the power of connection and community needed in order to ensure health care providers offer effective resources to not only support the health of people who are returning to our communities but to also ensure they thrive after reentry.
