On October 15, 2025, the Foundation for Health Care Quality and the Bree Collaborative presented Community Health Plan of Washington with the 2025 Mountain Climber Award for Excellence in Health Equity for CHPW’s equity-driven approach to improving diabetes care.
The Mountain Climber award is given for exemplary implementation efforts that focus on the Bree pillars of transformation. A multidisciplinary team from CHPW’s Quality, Care Management, and regional care teams implemented innovative strategies like at-home A1c testing, regional community outreach, and Community Health Center partnerships to address disparities across geography, race/ethnicity, and language.

“We’re honored to receive the Bree Collaborative Mountain Climber Award for Diabetes Care. This award recognizes our team’s dedication to supporting our members with culturally appropriate education and dedicated resources, affirming that we’re meeting them where they are with high-quality, equitable care,” said Edie Shreckengast, Health Coach in Care Management, who is part of the Diabetes Care Program, and accepted the award on behalf of the team.
Building on the momentum from receiving the Foundation for Health Care Quality and the Bree Collaborative’s Trailblazer Award for its Diabetes Care Program in 2024, this year’s Mountain Climber Award further affirms CHPW’s unwavering commitment to improving health outcomes for all Washingtonians through innovative, whole-person care.
