We’re celebrating National Case Management Week! This year’s theme is Encouraging, Engaging & Empowering, which is fitting for our incredible CHPW Care Management team that supports member-centered care.
Care Management is a process of building rapport with members and caregivers to assess and identify needs, collaborate with care teams to eliminate barriers to care, educating on health conditions, treatments and medications to support members in self-management and resources, and connecting members to resources to minimize social drivers of health.
