Celebrating National Case Manager Week

We’re celebrating National Case Management Week! This year’s theme is Encouraging, Engaging & Empowering, which is fitting for our incredible CHPW Care Management team that supports member-centered care.

Care Management is a process of building rapport with members and caregivers to assess and identify needs, collaborate with care teams to eliminate barriers to care, educating on health conditions, treatments and medications to support members in self-management and resources, and connecting members to resources to minimize social drivers of health.

Thank you to our Care Management staff for the challenging and rewarding work that you do every day!

This includes our:

Care Coordinators | Community Health Workers | Health Coaches

Carceral Transitions Case Manager | Complex Discharge Case Managers

Clinical Program Managers | Program Coordinators

Patient Review and Coordination Case Managers

Maternal Child Health Case Managers | DSNP Case Managers

Medicare Case Managers | Member Engagement Specials

Clinical Care Coordinators | Regional Case Managers | Field Case Managers

Medicaid Case Managers | Care Advocates | Care Management Leaders

Our Care Management staff work diligently with CHPW teams, CHCs, and community-based organizations to coordinate care for members.

Care Management staff strive to support and empower members to manage their health to optimize health outcomes and improve quality of life.
